Getting your supporting documents ready
Check the right checklist. Requirements depend on the service and the person or business applying. Use the checklist for your request; BIAI will confirm any exceptions.
Use valid, complete documents. Show both sides of identity cards where applicable and the photograph/details page of a passport. Include every page of the address document requested, dated within the required period—usually three months.

DrivePal helps monitor driving behaviour, including speed, acceleration, braking and cornering, and can support fleet enquiries. Sagicor General may require installation as a condition of particular cover, including some under-25 driver, fleet or hired-car arrangements; BIAI will confirm your policy’s requirement. Two valid certified IDs for the assignee (the person receiving ownership), and one valid certified ID for the previous policy owner. Include the front and back where applicable. If an executor or administrator is acting, BIAI will confirm their identity and authority documents.
One proof of address for the assignee dated within three months of submission. Send complete pages; fixed utilities, reputable charge-account statements, bank/credit-union or credit-card statements may be used. Mobile-phone bills and correspondence from another insurer are not accepted.
If using a spouse’s or parent’s address, bring their current proof of address and ID plus their letter confirming that you live with them. Both of you must sign in the advisor’s presence; bring originals for verification.
Letters of Administration or Probate, as applicable, if an estate representative is acting.
Review and sign the assignment and customer/tax-information forms supplied by BIAI. The owner and assignee must sign as required; BIAI will guide you on witnessing and any extra information needed if new benefits are requested.
Pay the confirmed processing cost: $10 for every $1,000, or part thereof, of the policy’s cash/fund value. BIAI will confirm Sagicor’s valuation, amount and payment arrangements.

Check the payments recorded for each Life policy using Sagicor GO or the client portal. Register first if you do not already have access; keep your policy number handy.
In the Sagicor GO app
Log in and select My Policies.
Find the policy you want to check and select Billing.

Choose the start and end dates. To review the whole policy, start from its inception date.
Select Retrieve.
On the Sagicor client portal
Log in and expand Life Insurance.

A payment looks missing?
Compare the policy history with your receipts or bank records for the same period.
Check when a direct debit or salary deduction actually started; an expected payment may not have been processed.
Changes to bank or branch details can also affect deductions. Ask BIAI to help investigate the specific transaction.
Have the policy number, payment date, amount, method and receipt or reference ready. Do not send your password or card details.

Baby’s birth certificate.
Certificate of Good Health from the paediatrician, noting height, weight and whether development is normal for the baby’s age.

State what percentage of the sum insured each beneficiary should receive.
State the following information for each intended beneficiary: First, Middle, Last National ID Number Birthplace Country of Residence Gender Residential Address Relationship to you the insured
For Sagicor Life, beneficiary shares must total 100%. For beneficiaries born in the USA or Canada, include their state or province of birth.
If a beneficiary’s name differs from the policy record, provide the applicable name-change evidence and identity documents BIAI requests. If a beneficiary is irrevocable, ask BIAI about the consents needed before changing the policy.

Marriage Certificate or Affidavit stating name change.
For Sagicor Life, provide applicable marriage/divorce documentation, birth certificate or affidavit supporting the change, plus one valid certified ID showing the new name. Tell BIAI whether the name belongs to the owner or the insured so we can supply the correct form.
A nominee can receive Critical Illness proceeds on your behalf if you are medically unable to do so, for example if you are in a coma, subject to the policy terms. Ask BIAI for the nominee form; we will help you arrange the change with Sagicor Life.

The deceased’s death certificate and the policy data schedule, policy specification or schedule of benefits (usually within the first four pages of the contract). Tell BIAI if the policy documents are missing.
Two valid photo IDs for the beneficiary or applicable estate representative, and one ID for the deceased. Include front/back where applicable. If using a birth certificate as alternative ID, ask BIAI to confirm acceptance; a birth certificate is not photo identification.
One proof of address dated within three months for the person receiving payment. For a local deposit, provide a bank statement matching the Direct Credit Authorisation Form; it may also serve as address evidence if current and showing your address.
Named beneficiary: review and sign the Claimant’s Statement, beneficiary customer/tax forms, the deceased’s tax form prepared in the estate’s name, and local Direct Credit Authorisation where applicable. Provide overseas Tax Identification Number evidence where required.
Minor beneficiary with no named trustee: court-approved documentation authorising payment to the parent or guardian.
Estate claim: provide Letters of Administration or Probate of the Will, as applicable, and the administrator/executor’s identity, customer/tax and payment information. BIAI will confirm the documents needed for the stage of the estate claim.
Estate funeral-payment request: provide the funeral-home invoice and review/sign the Payment of Proceeds Form, which also needs the funeral-home representative’s signature. An early release of up to $10,000 may be available only where the designation is Estate, subject to Sagicor’s approval and required documents.
If death occurred overseas: complete the Foreign Death Questionnaire supplied by BIAI and provide the burial or cremation certificate.
If you live overseas and request a wire transfer: provide address/residency evidence no older than three months, notarised or certified as Sagicor requires, and bank-issued wire instructions with bank name/address, account number, SWIFT and routing number. The local Direct Credit Authorisation and its matching bank statement are not required for this wire route.
BIAI will confirm whether a Proof of Death Physician’s Statement is required and guide you in obtaining it from the deceased’s physician. It is not required for every policy.

When a full medical is being completed at the same time, the panel physician manages the Chain of Custody process. BIAI will confirm the appropriate approved provider.

For approved individual health applications, a premium paid on or before the 15th of the month activates coverage from the 1st of that month.

Document requirements depend on the Sagicor Life service and person involved. Ask us before arranging certification or signing a form if you are unsure.
What to have ready
The service you are applying for and the identity/address documents you already have. Send complete pages and both sides of cards where applicable.
Address evidence should be dated within three months of submission. Include the second page of bundled-service bills where it shows the service address.
If using a spouse’s or parent’s address, bring their current proof of address and ID plus their letter confirming that you live with them. Both of you must sign in the advisor’s presence; bring originals for verification.
Tell BIAI if you have only one ID, an ID nearing expiry, overseas documents or questions about tax forms. We will confirm the applicable alternatives, witnessing and correct forms before you sign.
Provide accurate information about tax residency and source of funds when requested. BIAI will guide you to the appropriate insurer forms for your case.
No medicals are required for this decrease request. BIAI confirms the revised premium.
