Guidelines for Completing Forms & Questionnaires

  • Answer every question. Do not leave any questions or fields blank.
  • If a question does not apply to you, write “N/A” (Not Applicable), No or None rather than leaving it blank.
  • Provide complete and truthful information. Do not omit information because you think it may be unimportant.
  • Where a question asks for details following a “Yes” answer, provide all requested details.
  • If you make a mistake, cross it out neatly with a single line, enter the correct information, and initial the change.
  • Sign and date everywhere required, ensuring your signature is consistent with your usual signature.
  • If you are unsure how to answer a question, contact us for guidance before completing that section. Do not guess.

Getting your supporting documents ready

  • Check the right checklist. Requirements depend on the service and the person or business applying. Use the checklist for your request; BIAI will confirm any exceptions.
  • Use valid, complete documents. Show both sides of identity cards where applicable and the photograph/details page of a passport. Include every page of the address document requested, dated within the required period—usually three months.
  • Bring originals when requested. BIAI will explain the certification or overseas notarisation needed for your case. A witness signature on a form is different from certification of an identity or address document.
  • Using a parent’s or spouse’s address? Ask BIAI about the confirmation letter, their address evidence and identification, and the required signatures. Do not sign in advance if the signatures must be witnessed.
  • Declare tax and payment information accurately. Complete the tax-residency and source-of-funds information requested for your service. If you are unsure which form applies, ask BIAI before completing it.

Before your first payment

  1. Complete the applicable forms and send them for review before submitting originals. Wait for approval and confirmation that your account is open.
  2. The initial subscription is BBD $500; later contributions start at BBD $100, preferably monthly or at another suitable time. Confirm the current minimum for your selected fund.
  3. For a wire transfer, request current instructions from BIAI. If you are outside Barbados, ask for the banking instructions appropriate to your location.
  4. If paying at a Sagicor cashier for a new account, ask BIAI to confirm the correct fund payment reference before visiting.

Making subsequent contributions

  1. Online bill payments: use CIBC Caribbean (formerly FCIB) or Sagicor online banking and confirm the correct mutual-fund payee and account reference.
  2. SurePay: pay online or at an outlet using the confirmed fund and account details.
  3. Bank payments: direct deposit or transfer through First Citizens, or a transfer from another commercial bank, using instructions confirmed for your account.
  4. In person: cash, card or cheque at Sagicor Belleville; cheque payments at Wildey. Confirm opening hours before visiting.

Send payment confirmation

  1. After a wire transfer, send BIAI the transaction reference number so we can assist with follow-up.
  2. After a direct deposit at First Citizens, send a copy of the deposit confirmation or slip.
  3. Include your name, selected fund, account reference, payment date and amount. Email info@blackmaninsure.com and retain your receipt.

Register for online access

  • If using a spouse’s or parent’s address: bring their proof of address and ID, and a letter confirming that you live with them. Both of you must sign the letter in the advisor’s presence; bring the originals for verification.

Change of Name

  • Corporate Customer Identity Form, including applicable officers, senior management, authorised signatories, proxies and beneficial owners. For private companies, include persons owning or controlling 10% or more.
  • Evidence of authority to act, such as the relevant resolution or power of attorney, where applicable.
  • For a nonprofit or other entity: its registration or constituent documents appropriate to its legal form.
Contents insurance — increase or decrease sum insured

Ask BIAI to review an increase or decrease in the sum insured. Sagicor General must confirm any change, its effective date and revised premium. BIAI will confirm the applicable form and any valuation or other evidence for your policy; sending this request does not change your cover.

DrivePal helps monitor driving behaviour, including speed, acceleration, braking and cornering, and can support fleet enquiries. Sagicor General may require installation as a condition of particular cover, including some under-25 driver, fleet or hired-car arrangements; BIAI will confirm your policy’s requirement. Vehicle-location access is optional under the supplied agreement and needs the policyholder’s written approval.   2. Select your Sagicor General policy and check the premium amount and due date.

  4. Save the payment confirmation.

Schedule automatic payments with PayandGO

  1. Log in to the app or client portal and open Schedule a Payment.
  2. Follow the prompts to choose the policy, dates and payment schedule.
  3. Review the schedule and keep track of confirmations. Contact BIAI if a scheduled payment does not appear.

Other ways to pay

  1. SurePay online: select Sagicor General Premium, add your policy number and pay through your registered account. At a SurePay outlet, you can pay by cash or cheque.
  2. Online bill payments: in Scotiabank, look for Sagicor General; in CIBC Caribbean (formerly FirstCaribbean), look for Sagicor General Premium. Add your policy number and check that you have selected the correct insurer.
  3. In person: visit the Sagicor Financial Centre, Building #2, Lower Collymore Rock, St. Michael. The supplied guide lists cash, cheque, debit card and credit card payments. Confirm opening hours before visiting.
  4. By phone: call Sagicor General on (246) 431-2800 and ask for the cashier. The guide lists credit card and Visa- or Mastercard-branded debit card payments.

After you pay

  1. Email your confirmation to info@blackmaninsure.com.

We’ll guide you through the claim with care. The documents depend on whether payment goes to a named beneficiary or an estate, and whether payment is local or overseas. You can contact us before the full package is ready.

  • The deceased’s death certificate and the policy data schedule, policy specification or schedule of benefits (usually within the first four pages of the contract). Tell BIAI if the policy documents are missing.
  • Two valid photo IDs for the beneficiary or applicable estate representative, and one ID for the deceased. Include front/back where applicable. If using a birth certificate as alternative ID, ask BIAI to confirm acceptance; a birth certificate is not photo identification.
  • One proof of address dated within three months for the person receiving payment. For a local deposit, provide a bank statement matching the Direct Credit Authorisation Form; it may also serve as address evidence if current and showing your address.
  • Named beneficiary: review and sign the Claimant’s Statement, beneficiary customer/tax forms, the deceased’s tax form prepared in the estate’s name, and local Direct Credit Authorisation where applicable. Provide overseas Tax Identification Number evidence where required.
  • Minor beneficiary with no named trustee: court-approved documentation authorising payment to the parent or guardian.
  • Estate claim: provide Letters of Administration or Probate of the Will, as applicable, and the administrator/executor’s identity, customer/tax and payment information. BIAI will confirm the documents needed for the stage of the estate claim.
  • Estate funeral-payment request: provide the funeral-home invoice and review/sign the Payment of Proceeds Form, which also needs the funeral-home representative’s signature. An early release of up to $10,000 may be available only where the designation is Estate, subject to Sagicor’s approval and required documents.
  • If death occurred overseas: complete the Foreign Death Questionnaire supplied by BIAI and provide the burial or cremation certificate.
  • If you live overseas and request a wire transfer: provide address/residency evidence no older than three months, notarised or certified as Sagicor requires, and bank-issued wire instructions with bank name/address, account number, SWIFT and routing number. The local Direct Credit Authorisation and its matching bank statement are not required for this wire route.
  • BIAI will confirm whether a Proof of Death Physician’s Statement is required and guide you in obtaining it from the deceased’s physician. It is not required for every policy.

  1. In person: visit Sagicor Life Inc. at the Sagicor Financial Centre, Building #2, Lower Collymore Rock, St. Michael. Give the cashier your policy number and pay by cash, cheque, credit card, Visa debit or Mastercard debit.
  2. Wire transfer: contact BIAI for the current transfer instructions. Include your name and policy number in the transfer notes so the payment can be allocated correctly.
  3. Email proof of payment to info@blackmaninsure.com so BIAI can confirm with Sagicor that the premium has been applied to your policy.

Confirm when your policy or plan begins

  1. Ask BIAI to confirm the commencement date and any outstanding requirements with Sagicor.
  2. Payment alone does not confirm that an application is approved or that cover has started.
  3. For Peace Assured, the policy’s 24-month waiting period still applies. If the insured dies during that period, only the premiums paid are refunded.

Subsequent premiums: more ways to pay

  1. Preferred option: register on the Sagicor client portal using your policy number and pay by credit card, Visa debit or Mastercard debit. The Sagicor GO app offers the same card-payment options.
  2. In person: return to Sagicor Life with your policy number and pay by cash, cheque, credit card, Visa debit or Mastercard debit.
  3. SurePay online: select Sagicor Life Premium, enter your policy number and pay using your registered bank account or Visa debit card. At a SurePay outlet, provide your policy number and pay by cash, cheque or Visa debit card.
  4. Scotiabank online banking: select Sagicor Life, add your policy number and pay from your savings or chequing account.
  5. CIBC online banking: select Sagicor Life Premium, add your policy number and pay from your savings or chequing account.

For subsequent premiums on an existing policy, choose the payment method that works for you. If this is your first premium, use the separate first-premium guide for your policy type. Confirm the amount, due date and any agreed instalment arrangement before paying.

Pay in the app or client portal

  1. Register or log in to Sagicor GO or the client portal using your policy details.
  2. Select your Sagicor Life policy and check the premium amount and due date.
  3. Follow the payment prompts using a credit card or Visa- or Mastercard-branded debit card.
  4. Save the payment confirmation.

Schedule automatic payments with PayandGO

  1. Log in to the app or client portal and open Schedule a Payment.
  2. Follow the prompts to choose the policy, dates and payment schedule.
  3. Review the schedule and keep track of confirmations. Contact BIAI if a scheduled payment does not appear.

Other ways to pay

  1. SurePay online: select Sagicor Life Premium, add your policy number and pay through your registered account. At a SurePay outlet, you can pay by cash or cheque.
  2. Online bill payments: in Scotiabank, look for Sagicor Life; in CIBC Caribbean (formerly FirstCaribbean), look for Sagicor Life Premium. Add your policy number and check that you have selected the correct insurer.
  3. In person: visit the Sagicor Financial Centre, Building #2, Lower Collymore Rock, St. Michael. The supplied guide lists cash, cheque, debit card and credit card payments. Confirm opening hours before visiting.

After you pay

  1. Email your confirmation to info@blackmaninsure.com and slibbprp@sagicor.com.
  2. Include the policy number, payment amount, payment date and method. If one payment covers several policies, explain how it should be allocated.
  3. Keep a copy of your receipt and check that the payment has been applied.

  • Your policy/application reference, the requirements BIAI has confirmed and your appointment availability.
  • Bring the forms and valid government-issued photo ID specified by BIAI or the clinic. Confirm any preparation instructions with the clinic before attending.
  • For standalone Nicotine, Marijuana or Cocaine tests from 14 October 2026: bring the Chain of Custody and Medical Underwriting Request forms plus valid government photo ID. Sign the Chain of Custody form in front of laboratory staff at specimen collection.
  • When a full medical is being completed at the same time, the panel physician manages the Chain of Custody process. BIAI will confirm the appropriate approved provider.
